WebThe home visiting program is based on the well-evidenced Community Asthma Prevention Program (CAPP) operated by Children’s Hospital of Philadelphia. CAPP provides training and certification for Education Plus Health’s community health workers. Partnering Medicaid and CHIP managed care organizations provide reimbursement for the services. WebMethods: The CHWs provided education and resources to families of children diagnosed with mild, moderate or severe persistent asthma. A pre and post-test design was implemented to evaluate if the CHW intervention improved the family’s ability to successfully manage their child’s asthma.
